Documents Required for School Registration in Broward County
Here is a list of situations and required documents that may help you prepare for school registration.
Please note: Since requirements may differ from school to school, it is recommended that you check with individual schools.

SITUATION	DOCUMENTS
If your child will be ENROLLED FOR THE FIRST TIME 1, 2, 2A and 6 are REQUIRED 3 Optional	1. Two Proofs of address, either a water, light, or phone bill and rent lease or mortgage deed with parent or legal guardian name.
If your child attended a BROWARD COUNTY PUBLIC SCHOOL LAST YEAR 1 Required 2, 2A, 3, 4, 5 and 6 Optional	2. Up-to-date SHOT RECORD on Florida form
If your child attended a BROWARD COUNTY PRIVATE SCHOOL LAST YEAR: 1, 2 and 6* Required 2A, 3, 4, 5 and 6 Optional *6 required for kindergarten and first grade	2A. Proof of PHYSICAL EXAM within past 12 months for those entering a Florida school for the first time.
If your child attended a PUBLIC or PRIVATE SCHOOL OUTSIDE OF BROWARD COUNTY, but in the STATE OF FLORIDA last year 1, 2 and 6* Required 2A, 3, 4, 5, and 6 Optional *6 required for kindergarten and first grade	3. Social Security Card Optional: If child is an immigrant the card cannot be required.
If your child attended a PUBLIC or PRIVATE SCHOOL OUTSIDE THE STATE OF FLORIDA last year 1, 2, 2A, and 6* Required 3, 4, 5, and 6 Optional *6 required for kindergarten and first grade	4. Last Report Card or transcript from previous school, if available (for appropriate placement).
	5. Standardized Test Score
	6. Birth certificate or other proof of birth date: Baptismal record or family bible record accompanied by an affadavit sworn to by the parent; insurance policy which has been in force for 2 years; passport; notarized statement sworn to by parent accompanied by a statement signed by a physician that he/she believes child to be of required school age.
If your child attended any school OUTSIDE OF THE USA, or if your child was born outside the USA 1, 2, 2A, and 6* Required 3, 4, 5, 6 and 7 Optional *6 required for kindergarten and first grade	7. Passport may be used for proof or birth but it cannot be copied, only viewed and marked on registration form as such.
